Drug Rehabilitation Requirement (DDR) Team

Phoenix Futures has a team of specially trained assessors for people who are
required to change their substance misuse or maintain the changes they have
already made.
We work alongside all other drug services and offer access to structured
support and counselling to stabilise and reduce drug use, and help to make
long-term changes in thinking and lifestyle.

We aim to make our service welcoming and accessible. Substance misuse can
affect all social, cultural and ethnic groups, and we recognise the diverse
needs of our service users. We will try to overcome any barriers that might
make it difficult to use our service. If we cannot meet your needs we will do
our best to find you alternative support.

Responsibilities

In partnership with the South Yorkshire Probation Service, Phoenix Futures DRR
Team has responsibility for the assessment, referral to appropriate agencies,
monitoring and tracking of progress in treatment of clients referred to us in
the legal process.

Clients referred to us will be assessed for suitability to
enter the DRR programme. In most cases there will be immediate referral to
those agencies in Sheffield whose services best suit the individual needs of
each client. In some cases the need to stabilise on prescribed medication will
take precedence and so referral will initially be made to S.P.A.R. (Single
Point of Assessment and Referral at Fitzwilliam Centre). A date will be agreed
for review so that additional referrals to identified agencies may be made to
address each client’s individual needs.

These include: – Starting Over, The PSI Teams of Phoenix Futures and Turning
Point, Kickstart, Breakthrough, Phoenix Futures/ Turning Point Crack Group,
Action Housing and Target Housing/Hostels. There are also a number of
specialised structured programmes which may be accessed if considered suitable
for individual clients. Appointments made with any agencies will form part of
a client’s care plan and will appear on client’s weekly diary sheets prepared
by the Probation Service.

Tracking and Monitoring

DRR Team Members and Admin staff are required to track the progress in treatment
of all clients assigned to a DRR programme.

This will involve regular contact with all agencies involved to confirm client
attendance, receive future appointment details and monitor/discuss progress. This
contact is essential for us to prepare reports and inform the South Yorkshire
Probation Service, as required by the partnership.
