We aim to ensure our referral and admissions process is as swift and efficient as possible in order to capitalise on an individual’s motivation to engage with residential rehabilitation.

The Support Team will take some basic details of the referral and ask some basic screening questions to ensure we are able to take the referral.

We will then send you the following brief forms for completion and return:

● Initial risk assessment and third party information
● Service User Consent Form
● FAST alcohol screening tool
● Circumstances, Motivation and Readiness (CRM) Scale

Once returned, these forms are passed onto a therapeutic worker, who will be your named contact throughout the service user’s programme. They will liaise with you, the service user and other relevant third parties to complete a Risk Assessment and Management Plan. This can be in person or by telephone whichever is most convenient. Once risk has been assessed, we will be able to inform you of the admission decision. Providing funding is in place, an admission date can be agreed at this point.
