The Specialist Community Forensic Team (SCFT) is a new service that has been developed in response to the NHS New Care Models to improve the care of service users within the forensic mental health system. The SCFT works with mental health offenders from the Essex area who are being cared for within medium and low secure mental health units, primarily in Essex.
The SCFT provides a period of ‘in-reach’ work for six to nine months prior to discharge with the aim of building a therapeutic relationship. This might be to engage care planning, relapse prevention work and addressing potential challenges with discharge.
Services offered/ Interventions:
Provide support, advice and care and are experienced in understanding substance misuse. Support with triggers, relapse, risks and harm minimisation as well as focusing on relevant issues around activities of daily living. Where substance misuse is identified Phoenix Futures will work alongside to identify vulnerabilities and work on relapse prevention plans, signposting and ultimately advocating as advocates on behalf of the service user.
We also offer an extensive training package providing all the tools to become an effective peer mentor and make a difference to the lives of others. Once trained as a peer mentor they can access other training opportunities within Phoenix Futures as well as the chance to gain experience by facilitating groups with staff members, being part of our service user forums and be an active member on our interview panels.
