The GP Shared care service is available in over thirty (30) GP practices across Lambeth.

How to Refer:
You can attend the service for a walk-in assessment at 9a.m., Monday to Friday, also your GP, hospital, services, families and friends can also refer for an assessment. If you’re not sure what you need or want to find out more about what is available, you can always ring ahead for a chat.
